Freckle Laser Sessions: Check for Melasma First

Whether freckle laser treatment takes one session or several depends less on session count and more on whether melasma is mixed in. A look at the distinction, what a 2023 clinical study found, and what to plan for a short visit to Seoul.

Hello, I'm Dr. Yeonjin Kim, director of UH CELL Clinic's Gangnam location. When patients ask me whether freckle laser treatment can be finished in a single visit, I tell them the real question isn't how many sessions it takes — it's what kind of pigment is actually on the skin.

Freckle Laser Sessions: Check for Melasma First

Freckles alone versus freckles with melasma

Freckles with clear, well-defined borders generally respond well to laser treatment. But when melasma is mixed in, the picture changes. A 2023 clinical study published on PubMed Central (U.S. National Library of Medicine) describes freckles as an autosomal dominant condition that darkens with sun exposure, appearing as small brown spots. The same study notes that patients with clearly defined freckles tend to see good results, while those with blurred borders or coexisting melasma are harder to treat. Using a short-wavelength laser at high energy in just one or two sessions on this kind of skin can actually darken melasma or trigger post-inflammatory hyperpigmentation. That's why, when melasma is present, a lower-energy approach spread across more sessions is generally considered instead.

What the staged-treatment study actually did

The study I'm referencing treated 109 women, aged 27 to 53, with Fitzpatrick skin types III to IV, who had freckles combined with melasma. They received a Q-switched 1064nm laser at low energy with a large spot size, every 10 to 15 days, for a total of 10 to 15 sessions. Throughout treatment, participants followed strict skincare: gentle cleansing and moisturizing, sun avoidance, and SPF 30 or higher sunscreen.

The results were notable but specific to this group: freckles cleared in essentially all cases (a 100% effective rate), while melasma improved in 39.4% of cases. At the six-month follow-up, no freckles had returned, though melasma recurred in three patients, and no one developed post-inflammatory hyperpigmentation. This is the outcome of one study in one patient population — it doesn't mean every skin type needs the same number of sessions or will see the same result.

Planning around a short visit

If you followed this study's schedule exactly, the full course would stretch across several months. A visit of a few days to Seoul can't fit that timeline, so it helps to set realistic expectations before you arrive: what this trip can address, and what needs to continue afterward. The diagnosis step — sorting out whether it's freckles alone or freckles with melasma — is what determines which part belongs to this visit and which part belongs to ongoing care.

At our Gangnam clinic, we design each treatment plan after an individual skin diagnosis rather than running a fixed package. Since the study also points out that freckles darken again with sun exposure, I'd recommend planning your sun-protection routine for after you return home as part of the same conversation. Our Gangnam location is on the 2nd and 3rd floors of the UH FLAT SIGNATURE Gangnam hotel, and we have Japanese, Chinese, and English-speaking staff on site for patients visiting from abroad.

Individual skin response and recovery time vary. Please consult with our medical staff for a diagnosis and treatment plan suited to your skin.
