Rejuran vs Exosomes: Check Approval Before Comparing Glow

Rejuran Healer and exosome injections both get compared for radiant, dewy skin, but they are not the same kind of product under Korean regulation. Rejuran is a Ministry of Food and Drug Safety (MFDS) approved Class IV medical device injected into the dermis, while exosome products sold in Korea are registered as cosmetics meant for topical use. Before ranking one against the other for glow, it helps to know which approval each one actually carries, and why your skin looks dull in the first place.

When a patient asks me whether Rejuran Healer or an exosome treatment gives better glass-skin results, I don't start with a ranking. The two products begin from different approval categories and reach the skin through different routes, and that difference matters more than which one photographs better.

What Rejuran Healer Is Approved to Do

According to Pharma Research's own product manual for Rejuran (MFDS approval No. 14-825), the active ingredient is polynucleotide sodium (PN), and the device is classified as a tissue-repair biomaterial. Its approved purpose is described as the temporary improvement of facial wrinkles in adults. The same manual states that safety and efficacy beyond the period studied in clinical research have not been confirmed.

The word "glow" does not appear in that approved indication. Yakup Shinmun, a pharmaceutical trade publication, reported that Pharma Research introduced Rejuran as a product injected into the dermis to improve the skin's internal environment. In my own consultations, I describe Rejuran less as a radiance treatment and more as a way to support the skin's underlying condition. Any glow effect follows from that, rather than being the labeled purpose.

Why Exosome Injections Fall Outside That Approval

Exosomes raise a different question, because most products sold in Korea were never approved as injectables at all. According to Dailypharm, Oh Yu-kyung, who was serving as commissioner of the Ministry of Food and Drug Safety at the time, told lawmakers during a 2022 National Assembly audit that exosome products are being sold as cosmetics. Doctors News (청년의사) reported that cosmetics can be registered in Korea without going through a drug approval process, and that injecting a cosmetic-grade product carries a risk of complications such as foreign body granulomas or scarring.

A Seoul Administrative Court ruling addressed this directly. The court upheld a three-month license suspension against a doctor who had injected a topical exosome cosmetic into a patient's face using a hand-injection technique. Outside Korea, the U.S. Food and Drug Administration's consumer alert on regenerative medicine products states that there are currently no FDA-approved exosome products. Taken together, these sources point to the same gap: an exosome product labeled for topical use carries no injection approval, in Korea or in the United States.

I treat that gap as a reason to keep exosomes in the category of topical skin care instead of injectable treatment, and I tell patients the same thing when they raise it in consultation.

Diagnose the Dullness Before Comparing Products

Two patients can both report dull, lackluster skin and still need completely different plans. Before recommending anything, we look at a few questions during consultation:

• Is the dullness coming from dehydration or a weakened skin barrier?
• Has the skin thinned or lost elasticity over time?
• Is the skin still recovering from a recent laser session or other procedure?
• How much recovery time is available before a flight home or another fixed date?
