HIFU or Shockwave for a Sagging Jawline?

| Input:

For a sagging jawline, HIFU devices such as Ultherapy apply focused heat at set depths, while extracorporeal shockwave delivers pressure waves with far less facial evidence behind it. The right pick depends on which layer is sagging, how much recovery margin a Seoul trip allows, and prior treatment history.

A sagging jawline gets discussed with two different tools: HIFU devices such as Ultherapy that focus ultrasound into heat at a set depth, and extracorporeal shockwave devices that deliver pressure pulses without heating tissue. The two work through different mechanisms, and the research behind them sits at very different stages. Which one makes sense depends on where the sagging starts and how much recovery time your Seoul schedule allows.

HIFU or Shockwave for a Sagging Jawline?

Hello, I'm Dr. Kim Yeonjin, medical director of UH CELL Clinic's Gangnam branch. When patients research jawline lifting, they usually come across both ultrasound-based and shockwave-based options listed side by side, and it isn't always clear how to choose between them. I'd rather start with what each energy actually does inside the skin than with a device name.

Heat From Ultrasound, Pressure Without Heat

HIFU concentrates ultrasound energy at a single focal point to generate heat. A submental and neck lifting protocol for the Ulthera System, posted on ClinicalTrials.gov, specifies transducers set to three depths, 1.5, 3.0, and 4.5 millimeters, designed to create coagulation zones of roughly 1 cubic millimeter within the deep dermis and the tissue layer beneath it. A histology study published in PubMed Central describes these coagulation points reaching temperatures above 60 degrees Celsius, after which a collagen remodeling response follows.

Extracorporeal shockwave is a mechanical stimulus: a very short, strong pressure pulse delivered to tissue. It has no thermal coagulation stage, which is the main mechanical difference from HIFU.

How Far the Evidence Actually Goes for the Jawline

Ultherapy's evidence base for the jaw area is comparatively well documented. A narrative review published in PubMed Central notes that Ultherapy first received FDA clearance in 2009 for non-invasive eyebrow lifting, with clearance for neck and submental lifting added afterward.

Shockwave's effect on facial skin is best described as an early research stage. A 2019 study in the Journal of Surgical Research applied a single low-energy shockwave session to the upper and lower jaw skin of goats and reported increased dermal thickness and collagen production four days later. The researchers framed this as a finding worth pursuing for surgical flap healing, not as a measurement of how much a sagging human jawline lifts. Carrying that result over to jawline lifting in patients is a bigger step than the study itself supports.

Three Things I Check Against a Seoul Schedule

  • Which layer the sagging starts in. If the laxity traces to skin elasticity rather than deeper support structures or fat distribution, that changes which focal depths matter. When a deeper layer looks involved, I look first at HIFU designs that can split treatment across multiple depths.
  • Pain tolerance and recovery margin. Heat-based treatments can bring temporary soreness or swelling for some patients, so I map that against the return flight date before finalizing a plan.
  • Recent treatment history. A recent lift or filler changes how I space energy delivery and where I place it.

Our Gangnam branch is equipped with Ultherapy PRIME, HIFU, and Thermage for lifting, and the specific settings and depths are decided after diagnosis, not before it. The branch sits on the second and third floors of the UH FLAT SIGNATURE Gangnam hotel, so patients can return to a hotel room to rest right after treatment, and Japanese-, Chinese-, and English-speaking staff are on site for consultations. I went into more detail on how device design interacts with contour and firmness in an earlier piece, V-RO Advance Lifting: Designing Contour and Firmness Together, for readers comparing energy-based options more broadly.

FAQ

Is a Shurink-type device the same treatment as Ultherapy?

Both belong to the HIFU family because they focus ultrasound energy, but they are different devices with different specifications. The Ulthera System's ClinicalTrials.gov protocol specifically notes real-time ultrasound imaging during treatment to visualize tissue to a depth of up to 8 millimeters.

If there's a lot of fat under the chin, is lifting alone enough?

If the sagging traces mainly to fat distribution rather than skin laxity, that usually needs a separate evaluation alongside any lifting plan. Diagnosis first separates the causes so the plan can address each one.

How should I plan a visit if my stay is short?

I set the scope of that visit based on the number of days available and the recovery margin remaining before your flight, then note what to continue on a future visit so the plan carries forward.

Skin response and recovery time vary from person to person. Please consult a physician for an accurate diagnosis and treatment plan before booking any procedure.
