Sagging and Wrinkles Come From Different Skin Layers
Sagging along the jawline and fine lines on the surface are not the same problem, and treating them with the same device usually disappoints one side of the result. Sagging is a deep-layer issue that responds to focused ultrasound reaching the SMAS, the fibrous support layer beneath the skin, while fine lines and lost elasticity sit in the dermis, where radiofrequency heat works best. Fine lines and a "thinner, looser" surface texture are a different story. They come from the dermis itself, the layer that manufactures the collagen and elastin responsible for skin's firmness and rebound. A dermis with reduced collagen density stretches and creases more easily even when the SMAS underneath is still relatively intact.
Because these are two different structures, a single energy setting rarely serves both well. This is the reason we ask a patient to describe sagging and fine lines as separate concerns, not one combined complaint of "aging skin," before we plan anything.
What a 2025 Review Says About Depth-Controlled Ultrasound
A narrative review published in the Journal of Cosmetic Dermatology in 2025 describes how microfocused ultrasound with real-time visualization is delivered at three fixed depths, 1.5mm, 3.0mm, and 4.5mm, corresponding to the papillary dermis, reticular dermis, and the SMAS. The review explains that the device raises tissue at each depth to roughly 60 to 70 degrees Celsius, producing discrete thermal coagulation points. Those points trigger a wound-healing sequence: an initial inflammatory phase, then fibroblast activity that produces new collagen and elastin, then a remodeling phase that continues for weeks to months after treatment.
That staged mechanism matters for how we set expectations. The lift a patient sees in the days after treatment is largely from the immediate contraction of heated collagen. The more durable change, the one that actually holds the jawline or brow position, builds gradually as new collagen matures, which is why we tell patients the three-month mark is a more honest checkpoint than the day of the appointment.
Merz Aesthetics, the manufacturer of the Ultherapy PRIME system, stated in a 2025 announcement that the device carries U.S. FDA clearance to lift the eyebrow, lift lax skin under the chin and on the neck, and improve lines and wrinkles on the décolleté. The same announcement listed the most common side effects reported in clinical trials as redness, swelling, pain, and transient nerve effects, meaning temporary changes in sensation that resolve on their own. We walk patients through this list before treatment rather than after, because a redness or swelling that appears on day one should not come as a surprise.
Where Radiofrequency Fits
Radiofrequency devices are generally used to heat the dermis directly rather than the SMAS, which is why we reach for them when the concern is fine lines, surface laxity, or the dermal thinning that shows up as crepiness, rather than a jawline that has genuinely dropped. The exact depth reached varies by device design, so we do not treat every radiofrequency platform as interchangeable when we plan a session.
In practice, this means we rarely treat sagging with radiofrequency alone, and we rarely treat fine-line texture with a full course of SMAS-depth ultrasound. When a patient has both concerns, and most patients over 40 do, the two technologies are often used in combination, with the ultrasound settings addressing the structural drop and the radiofrequency pass addressing surface quality.
Why We Diagnose Before We Choose a Device
Our consultation records sagging, wrinkling, and elasticity as three separate measurements, not one overall impression, and the treatment plan follows from those numbers rather than from a fixed package. What the Hours After Treatment Look Like
Both ultrasound and radiofrequency generate heat, and it is normal to see redness and mild swelling for several hours afterward. Sometimes. Because they sit at different depths, ultrasound and radiofrequency can be scheduled on the same day or split across visits depending on how much recovery time a patient wants between sessions. We decide this after the diagnostic measurements, not before.
How soon will I see results?
The 2025 Journal of Cosmetic Dermatology review describes an immediate collagen contraction followed by new collagen formation over weeks to months. We ask patients to judge the result at the three-month mark rather than the day after treatment.
Can this be done during a short trip to Seoul?
