Fractional lasers create microscopic treatment zones, allowing the surrounding skin to support recovery. CO₂, Er, picosecond, and non-ablative fractional laser systems differ in treatment depth and thermal effect. I select the appropriate system based on scar type, skin tone, and acceptable downtime—not by device name.

Deeper is not always better

CO₂ fractional lasers can induce significant remodeling but involve more redness, crusting, downtime, and a higher risk of post-inflammatory hyperpigmentation. Deep, tethered scars may also require subcision or focal treatment rather than repeated laser treatment alone.

Erbium balances precision and recovery

Er lasers can ablate precisely with less thermal spread, but a gentler treatment is not sufficient for every scar. Energy settings and the number of passes must match the skin thickness and scar depth.

Pico fractional treatment suits texture concerns and superficial scars

Pico fractional systems can improve the appearance of enlarged pores, uneven texture, and superficial scars with shorter downtime but cannot fully correct deep rolling or boxcar scars when used alone.
