Managing acute and delayed filler complications

Adverse reactions after filler treatment require different responses depending on when they occur. Vascular complications immediately after injection require urgent assessment, while swelling or firmness that recurs months or years later may indicate a delayed inflammatory or immune response.

Do not wait on unusual pain or color change

Severe pain, pallor, mottled skin, or visual symptoms may indicate vascular compromise. Contact the treating clinic immediately rather than assuming it is simple bruising. Prevention depends on anatomical knowledge, precise injection depth, conservative dosing, and an established emergency protocol.

Recurrent delayed swelling requires evaluation

An area may swell again during periods of fatigue, infection, or stress after initially appearing normal. If the tissue becomes firm or the contour changes, the product used, injection plane, and dose should be reviewed, and both inflammation and infection should be assessed.

Avoid repeated stimulation in the same area

Injecting additional filler while the previous product remains, performing other injections at short intervals, or using vigorous massage can place additional stress on the tissue. The location and condition of existing filler should be assessed before another treatment.

Consider immune status and use the minimum effective dose

I am Dr. Kyungsoo Kim. A history of allergies, autoimmune disease, recent infection, and current medications should be reviewed during the consultation. I use the minimum amount required and allow sufficient intervals between treatments. Early recognition and consistent documentation are central to the safety of repeat treatments.
