Why Do Eye Wrinkles Need Three Different Treatments?

Eye wrinkles are not one problem with one fix. Eye wrinkles split by layer, not by name

The question we hear most often in consultation is simple: what's best for the eye area. Our answer starts with a different question. Does the line only show up when you smile? Does it stay when your face is completely still? Or is it more of a shadow that makes you look tired even with no lines at all? Each answer points to a different origin, skin, muscle, or volume, and that origin decides what we recommend.

Lines that only show when you smile

This type comes from repeated contraction of the orbicularis oculi, the ring of muscle around the eye. A 2025 ultrasound study of 52 healthy adults measured this muscle at an average thickness of about 1.56mm, ranging from 1.03mm to 2.31mm. A muscle that thin folds the skin above it every time you close your eyes or smile.

According to the FDA's prescribing information for botulinum toxin products, these formulations are indicated for the temporary improvement of moderate to severe lateral canthal lines associated with orbicularis oculi activity, and the most commonly reported adverse event at the injection site is eyelid edema, at about 1%. The same labeling notes that safety and efficacy have not been established for dosing more frequently than every three months. That is why we confirm the interval between sessions before anything else in consultation.

Fine lines that stay even when your face is still

The skin around the eyes is the thinnest on the face. A 2015 study measuring dermal thickness across facial sites found the upper eyelid's inner region had the thinnest dermis, at roughly 759 micrometers, less than half the thickness measured beside the nose. There is almost no subcutaneous fat in this area either, so a drop in collagen and moisture tends to show here before it shows anywhere else.

Because this layer isn't driven by muscle contraction, relaxing the muscle doesn't do much for it. The candidates instead are treatments that stimulate the dermis directly or add material back into it. A 2026 prospective observational study in the Journal of Cosmetic Dermatology followed 42 patients who received periorbital polynucleotide injections and found that patient-reported scores for lower eyelid and periorbital wrinkles improved from baseline at the one, three, and six month marks, with satisfaction peaking three months after two sessions. It's an observational design, not a controlled trial, so it doesn't prove the effect on its own, and results vary by individual.

Local practice patterns matter here too. A 2024 survey of Korean dermatologists published in the same journal found periorbital and under-eye fine lines ranked among the top indications for polynucleotide injection, with a three-session course spaced four weeks apart reported as the most common protocol. Shadows under the eyes that aren't about lines

When two layers overlap

In practice, expression lines and static fine lines often show up together. When that happens, we don't treat both at once. The same layer-first logic applies to cheek sagging and jawline changes elsewhere on the face: the layer involved, not the area's name, decides the treatment.

Can heat or ultrasound lifting treat under-eye hollowing too?

When can I wear glasses, contact lenses, or makeup again?

This depends on the specific treatment, so we go over it in detail in person. Across all treatments, sun protection and moisturizing afterward matter, and it helps to reduce anything that irritates the treated area in the days after.
