Seoul Skin Clinic Fees: Why the Total Quote Matters Most

Consultation fees at Seoul dermatology clinics range from free to a paid charge because no single Korean law sets one standard fee. What actually decides your budget is whether that consultation ends with every treatment item, session count, and price locked into one VAT-inclusive total. Since January 1, 2026, that total also includes the 10 percent value-added tax that foreign patients used to be able to claim back.

When a patient asks me how much the consultation costs, I usually turn the question around and ask what they actually want confirmed by the end of it. The fee itself tells you very little about what happens once you sit down.

Why Consultation Fees Vary So Much

Cosmetic dermatology procedures in Korea fall outside national health insurance coverage, so clinics set their own consultation policies rather than following a fixed schedule. Some offer a complimentary consultation to bring patients in the door. Others charge a fee, sometimes credited toward treatment if the patient proceeds, sometimes not. Both practices are legal. A free consultation is not automatically more generous, and a paid one is not automatically more thorough. The policy is a business decision the clinic makes on its own; it says nothing about what gets disclosed once you are in the room.

What Korean Law Actually Requires Clinics to Disclose

Whatever the consultation costs, Korean law is specific about what must happen before treatment begins. Article 45 of the Medical Service Act requires medical institutions to make the price of non-covered, or bi-geubyeo, procedures known to patients and prohibits charging more than the amount disclosed. Following a 2020 revision to the Enforcement Rule of the Medical Service Act, the Constitutional Court, in its 2021Hun-Ma93 decision, confirmed that clinics offering procedures designated by the Ministry of Health and Welfare must explain the item and its price directly to the patient before treatment starts. That obligation stands regardless of whether the initial consultation carried a fee.

In practice, this means the consultation is where the treatment plan, the number of sessions, and the price per session should be put in writing before anything gets scheduled. A figure mentioned over email or through a booking app is not the same as a written estimate handed to you during the visit. I ask patients to treat the verbal quote as provisional until they see it printed on paper or on screen with their name attached.

The VAT Change That Took Effect This Year

A second shift changed what "total price" means for visiting patients specifically. Under Article 107-3 of the Restriction of Special Taxation Act, foreign tourists undergoing cosmetic medical procedures at registered institutions could reclaim the 10 percent value-added tax charged on the service. As The Korea Herald reported in December 2025, that special provision expired on December 31 of that year, after the National Assembly's year-end tax revision left the extension out of the final bill. From January 1, 2026, foreign patients pay the same 10 percent VAT as domestic patients, with no refund path currently in place.

Health Kyunghyang, reporting in 2026 on data from Rep. Lee Dal-hee's office at the National Assembly, put the average per-case refund over the program's decade of operation near 110,000 won. That is roughly the amount now folded permanently into the invoice instead of coming back afterward. A bill from Rep. Lee Dal-hee, also covered by Health Kyunghyang in 2026, proposes reapplying the refund through December 31, 2029. As of this writing it remains a proposed amendment, not enacted law. Until it passes, I tell patients to build a Seoul procedure budget around the VAT-inclusive figure rather than assuming a refund will return in time for their trip.

What This Means for a Short Medical Trip

For a patient staying in Seoul for a matter of days, the gap between a rough verbal quote and a written VAT-inclusive total is not a minor detail. It is the difference between arriving with a workable budget and discovering an unexpected balance at checkout, with little time left to renegotiate or shop around. Session-based treatments compound this: if a plan calls for three sessions rather than one, the price per session matters far more than the headline number quoted for a single visit. A written estimate should state the per-session price and the total across every planned session, not just an opening figure.

Three Things to Confirm Before You Book

I ask patients to leave a consultation, paid or free, with three specific answers rather than a general impression:

• The treatment items recommended, how many sessions each requires, and the price per session
• Whether that price already includes the 10 percent VAT or is quoted before tax
• Whether the clinic accepts foreign-issued cards and can issue a receipt for that payment

If a clinic cannot answer the second question clearly, ask for the VAT-inclusive total in writing before agreeing to a schedule. That single line is what keeps a short trip from running over budget on the last day.

If I already paid a consultation fee, is it deducted from the procedure cost?

That depends on the clinic's own policy, so confirm it when you book. At our clinic, we present the VAT-inclusive total for the recommended plan during the same consultation, regardless of whether a separate consultation fee applies.

Could the VAT refund come back before my trip?

A bill to reinstate the refund through 2029 has been proposed in the National Assembly, as Health Kyunghyang reported in 2026, but it had not passed as of this writing. Until a refund program is actually reinstated, budgeting with the 10 percent VAT included is the safer approach for a Seoul procedure.

What should a written estimate actually include?

It should list each recommended treatment item, the number of sessions it requires, the price per session, and whether that price is stated before or after the 10 percent VAT. A verbal figure without those four elements is not yet a usable budget number.
