If Thermage Feels Ineffective, Check the Goal Before the Device

For people researching Korea travel, Korean dermatology and Gangnam skin clinics, Thermage is often understood as a representative tightening treatment. In my consultation room, when a patient says the Thermage did not work, my first question is what result they were expecting: skin texture, lifting, fat reduction or facial contouring. Thermage is an RF device for dermal tightening and texture; it is not primarily a fat-reduction treatment.

Thermage reorganizes dermal collagen

Thermage delivers radiofrequency heat to the dermis, causing collagen contraction and regeneration signaling. Some firmness may be felt early, but meaningful collagen remodeling usually develops over one to two months. That is why Thermage should be scheduled before an important event rather than immediately beforehand.

Ulthera and Thermage are not the same lifting treatment

Ulthera targets deeper support layers such as the SMAS and is used more for lifting. Thermage works more on dermal texture, elasticity and fine lines. If a patient expects Ulthera-like lifting from Thermage alone, disappointment is likely. On the other hand, thin skin with fine lines and poor texture may benefit significantly from Thermage.

Fat and volume concerns need separate planning

When facial fullness or sagging is caused by descended volume, Thermage alone may not be enough. Ulthera, fat-dissolving injections or devices such as ONDA may be considered. Conversely, when the face is lean and the skin is thin, RF energy must be designed carefully.

The key to Thermage results is not the device name but the diagnosis. We must separate whether the goal is skin texture, lifting the SMAS layer or fat reduction before choosing the right combination.
