Hello, I'm Dr. Kim Yeonjin, director of UH CELL Gangnam. When patients research melasma laser treatment in Korea, the number of sessions and the total cost usually come up together. I explain both from the same starting point: the type of pigment and how far it has spread decide the session count, and the session count decides the cost.

How Many Sessions Does the Research Show?

In a split-face study published in Dermatologic Therapy in 2023, Lee and colleagues treated 24 melasma patients with a low-fluence 1064 nm picosecond laser on one side of the face and a low-fluence Q-switched Nd:YAG laser on the other, five sessions at two-week intervals. Of the 21 patients who completed the study, both sides showed a meaningful drop in the melasma severity index (mMASI) and melanin index starting from the second week, with no adverse effects reported.

In a separate study in Lasers in Surgery and Medicine in 2021, Wong and colleagues treated 20 Asian patients with a fractional 1064 nm picosecond laser, nine sessions spaced four to six weeks apart. The average mMASI score fell from 10.8 at baseline to 2.7 six weeks after the final session. Both studies involved small groups of twenty to twenty-four participants, and a lower index score means pigment has lightened, not that it has disappeared completely.

Why Melasma Needs Multiple Sessions

Lee's team noted that even low-fluence toning can cause side effects if recovery time between sessions is too short or if repeated treatment pushes cumulative energy too high. Rebound pigmentation is a known risk in Asian skin types III to IV, which is why melasma toning is planned at a lower fluence and spaced out rather than compressed into fewer, stronger sessions. Shallow, epidermal freckles follow a different session plan, since that pigment sits closer to the surface. I cover that separately in consultation rather than folding it into a melasma schedule.

Planning Sessions and Cost Around a Short Visit

A five-session schedule at two-week intervals takes at least eight weeks from the first treatment to the last. That timeline does not fit inside a single short trip, so for patients visiting from abroad I recommend completing a diagnosis and first session on this visit, reviewing how the skin responds after returning home, and then planning the next visit around that response.

Cost follows the same logic. Comparing only the price of one session leaves out the number of sessions your pigment actually needs and the travel schedule required to complete them. During consultation, I go over an expected session range and spacing so you can estimate the total cost before committing to a plan.

Is picosecond toning better than Q-switched toning for melasma?

In Lee's split-face study, both treated the same five-session, two-week schedule, and the picosecond side showed a slightly larger drop in mMASI, but the difference was not statistically significant. The fluence and interval matched to your pigment matter more than the device name.

Does melasma come back after finishing all the sessions?

In Wong's study, the average mMASI was 2.7 six weeks after the last session, then rose to 3.6 by twelve weeks after. Because of that, I build a maintenance plan into the consultation for after your sessions are complete, not just the initial schedule.

What if I have both melasma and freckles?

Each pigment type needs its own session plan and fluence setting. Diagnosis comes first to separate the two, and that determines which pigment we address first.
