The same procedure can cost a different amount out of pocket depending on tax treatment and how you booked your appointment, and that difference is worth understanding before you commit to a date.

Why the math changed for 2026

For nearly a decade, foreign tourists who paid for cosmetic and dermatological procedures in Korea could claim back the 10 percent value-added tax charged on the service. The Ministry of Economy and Finance announced in its 2025 tax reform plan that this refund would not be extended, and the National Assembly's final revision of the tax restriction law left the extension clause out entirely. The refund had already been renewed six times since it began in 2016, and this time lawmakers let it lapse. Korea Medical Communication reported in 2025 that the refund ran roughly 100,000 to 200,000 won per patient depending on the procedure, money that is no longer coming back on any treatment paid for from January 1, 2026 onward.

There is one narrow exception worth knowing if you are reading this early in the transition: eligibility follows the date you paid, not the date of your treatment. A patient who settled the bill in 2025 and departs Korea within the refund window can still claim it even if the procedure itself takes place in 2026. Anyone paying now should assume the refund does not apply and budget the full tax-inclusive amount.

What else can be sitting inside the quoted fee

Several other line items shape the total as well. Korea's Ministry of Health and Welfare caps the commission a licensed medical institution may pay a patient-referral broker at 30 percent of the total treatment fee. If you booked through an agency or app rather than contacting a clinic directly, that commission can already be folded into the number you were quoted, which is one reason the same procedure can carry different prices depending on the booking path. The same government notice draws a firm line around what counts as a referral commission in the first place: interpretation, local transportation, sightseeing, and lodging are explicitly excluded from it. In practice, that means these costs are usually billed to you separately from the treatment fee, not absorbed into it, so a complete budget has to add them on top rather than assume they are already covered.

Lee Dal-hee, a member of the National Assembly's Health and Welfare Committee, has proposed an amendment to the tax restriction law that would restore the refund, Dental News reported in August 2026. The proposal is still at the discussion stage, so it is safer to plan your budget as if no refund applies and treat any reinstatement as a bonus rather than something to count on.
